Privacy policy

Ensemble Innovation Ventures (“Ensemble”), respects the privacy rights of individuals who visit, use, and interact with (collectively “Users”) the Ensemble Website (“Ensemble Website”).  Information may be collected from Users during their visits to the Ensemble Website to allow Ensemble to provide better service. Ensemble is concerned about treating this information with care, so Ensemble has implemented this Privacy Policy (“Privacy Policy”). This Privacy Policy has been updated effective February 8, 2021.

Types of Information Collected and How it is Used

Aggregate Information: Ensemble may collect information that is not personal information about Users to measure traffic on the Ensemble Website. This information is collected automatically on an aggregate basis (“Aggregate Information”) through the use of a “cookie” or small text file placed on the User’s hard drive by the User’s Internet browser. The cookie allows Ensemble to track the number of visits to various pages on the Ensemble Website. The cookie is used only during a single connection to the Ensemble Website. The cookie is not used to track User activity after leaving the Ensemble Website. The cookie is permanently disabled when the User exits the Ensemble Website. Aggregate Information helps Ensemble understand Users’ needs, improve the Ensemble Website, and demonstrate to others the volume and nature of Ensemble Website traffic. Aggregate Information may be shared with third parties as described in the section called “Information Provided to Third Parties” below.

User Information: Information about individual Users is not collected, saved, or distributed to others by Ensemble except as provided for in this Privacy Policy. User information from the Ensemble Website may be obtained, retained, and/or used (collectively “Used”) by Ensemble to 1) provide Ensemble’s Services, including but not limited to facilitating Users’ access to, use of, and/or participation in any products, services, resources, or surveys (collectively “Services”) from the Ensemble Website; 2) manage User accounts; 3) to facilitate a contract or other agreement between Ensemble and User; 4) contact any User; 5) provide Users with news, special offers, or other information about goods, services, events, or other items offered by Ensemble that are similar to those you have purchased, accessed, or otherwise indicated interest in; 6) manage User requests; and/or 7) other purposes, including but not limited to data analysis, identifying usage trends, determining the effectiveness of advertising and communications, and evaluating and improving our Services, marketing, and experience. Information about Users may be provided by Ensemble to other entities, such as insurance carriers, vendors, healthcare entities, banks, venture capital firms, investors, and other parties providing Services through the Ensemble Website (collectively “Service Providers”). Such information may be Used by Ensemble and Service Providers.

Services Information: Information from Users on the Ensemble Website may be Used by Ensemble, for a variety of reasons, such as, but not limited to, for processing requests for and/or participation in Services offered by or through Ensemble, including, but not limited to, from Service Providers. Such information about Users also may be provided to and Used by Service Providers, as described in the section called “Information Provided to Third Parties” below.

Email Information: Information that Ensemble receives from Users via email may be Used by Ensemble and may be provided to and Used by third parties as described in the section called “Information Provided to Third Parties” below.

Information about Children Under 17: The Ensemble Website is intended for adults participating in, disrupting, improving, and/or innovating the health and wellness industries. It is unlikely that children under the age of 17 will use the Ensemble Website. It is Ensemble’s policy not to collect personal information of children under age 17.

If it is necessary to collect such personal information from a child identified as being under age 17, Ensemble will do so only after obtaining verifiable parental consent. If Ensemble becomes aware that Ensemble has inadvertently collected personal information from a child under age 17 without parental consent, such personal information will be deleted from Ensemble’s system.

Information Provided to Third Parties

Information collected on the Ensemble Website will only be provided to third parties in the manner specified in this Privacy Policy. Persons who do not want their information shared in this manner should not provide the information.

Information Used by Ensemble may be provided to third parties, such as, but not limited to, Service Providers. Information shared with such third parties may be Used to: (i) conduct business, including, without limitation: (A) completing any transaction(s) requested by the User; and (B) serving Users; and/or (C) reporting on transactions, activities, and Services conducted with or facilitated by Ensemble.

Ensemble also may disclose such information to: (i) comply with the law or legal process; (ii) exercise Ensemble’s legal rights or defend against legal claims; (iii) enforce Ensemble’s policies (including, without limitation, Ensemble’s Terms of Use); and/or (iv) protect the rights, property, or safety of others.

Security

Ensemble uses a combination of industry-typical security measures to protect personal information Ensemble collects. Such measures will change from time to time. Ensemble makes no representations or warranties of any kind, express or implied, that such measures will protect all personal information collected. Ensemble will make reasonable efforts to protect such information from unauthorized access.

Links to Other Websites

The Ensemble Website may, from time to time, contain links to websites of Service Providers, other Ensemble partners/vendors, and other businesses and resources (collectively “Linked Resources”). Each Linked Resources website may have its own privacy and data collection policies and practices. Ensemble is NOT responsible for the privacy and data collection policies and practices of any Linked Resources or for the content of the Linked Resources websites. Users interested in the privacy and data collection policies and practices of Linked Resources should review the policies of those Linked Resources websites Users choose to access.

Links to other websites, including, but not limited to, Linked Resources websites, do not imply Ensemble’s endorsement or approval of such websites or the resources and information contained within those websites, nor are such links or references indications that Ensemble has received specific authorization to provide these links or resources. Ensemble does not endorse, approve, certify, or control such external websites. Ensemble is not responsible for the accuracy, timeliness, completeness, efficacy, merchantability, usefulness, fitness for any particular purpose, or correct sequencing of information located at such websites.

The links and references on the Ensemble Website to other websites, including, but not limited to, Linked Resources websites are provided solely as a convenience to Users of the Ensemble Website.

Ensemble makes no representations or warranties of any kind, express or implied, about the Linked Resources websites. Ensemble disclaims all warranties and responsibilities of any kind, including, without limitation, warranties and responsibilities with respect to Linked Resources’ websites, content, privacy, and data collection policies and practices, and actions.

Your Rights to Access and Amend Your Personal Information

You have the right to request access to the personal information, if any, that is maintained about you on the Ensemble Website to ensure that such personal information is accurate. Subject to certain exceptions required by law, and, provided Ensemble can authenticate your identity, you also can request corrections, amendments, or deletions of any personal information obtained through the Ensemble Website or otherwise in Ensemble’s possession as appropriate. You may also request changes to your preferences regarding how Ensemble uses or discloses your information or how Ensemble communicates with you.

To obtain access to, or seek to correct, amend, or delete any of your personal information, or for any other questions or concerns about Ensemble’s personal data policies and practices: Submit a request in writing to Ensemble. The request must include: (i) your name; (ii) your address; (iii) your telephone number; (iv) the specific personal information at issue; (v) the identity of the document or record that contains the disputed personal information; and (vi) the desired correction, amendment, or deletion (along with any appropriate supporting documentation). Within thirty (30) business days after Ensemble receives your request and all required information, Ensemble will notify you whether: (i) Ensemble requires additional information; (ii) Ensemble has made the requested correction, amendment, or deletion; or (iii) that Ensemble cannot make the requested correction, amendment, or deletion, and the reasons for Ensemble’s decision.

Privacy Policy Changes

Ensemble reserves the right to and may change this Privacy Policy at any time without notice to you. Changes will become effective when the revised Privacy Policy is posted on this Ensemble Website. Ensemble may email periodic reminders or notices about changes. You are encouraged to review this Privacy Policy frequently, so you are informed about Ensemble’s privacy practices and policies.
